Albus Dumbledore and the Chamber of Secrets
In his second year as headmaster of Hogwarts, Albus Dumbledore raises the bar and sets a new record for wilful incompetence in the workplace. His negligent disregard for the safety of staff and students is obscene. A parent governor valiantly tries to alert the authorities when Dumbledore fails to show any interest in substantiated reports that a 200 foot long magic snake is loose in the school. It is only after several students are paralysed and one goes missing entirely, that Dumbledore finally reacts. By going on holiday.

In Dumbledore's defence, he does return before the end of the school year, to congratulate the twelve year old students who have once again bailed him out. It's incredible that no punitive action is taken against Dumbledore for abandoning his duty of care.

The second Harry Potter film sees Dumbledore failing to take action after an ancient, extraordinarily large, phenomenally dangerous creature is released inside the school. Even after a number of students are paralysed by the creature, Dumbledore doesn't seem to express any proportionate level of concern. It's difficult to imagine what you would really do in this situation, and it's understandable that there is no established procedure that is applicable. Immediate evacuation, however, would seem a no-brainer. Although, to be fair, that would probably make for a fairly dull film.
